4. Proof of Identity – Copy of Aadhaar Card and 2 Passport Size Photos
5. SSLC / 10th Equivalent & Plus Two / 12th Equivalent Certificates
6. Original Mark Lists of MBBS
7. MBBS Degree Certificate
8. CRRI / CRMI Certificate
9. Fee Receipt Number (for those who paid fees at CEE)
10. TCMC Registration (MBBS)
11. Migration & Eligibility Certificates (or Equivalency Certificate)
12. Transfer Certificate (from last studied college)
13. Course & Conduct Certificate
14. Relieving Order (from 1st Counselling Service)
15. Community Certificate / Non-Creamy Layer Certificate / EWS Certificate (if applicable)
16. Passport Size Photos – 2 Copies
17. Vaccination Certificates (Hepatitis A & B, Chickenpox, and MMR)
18. Physical Fitness Certificate
19. Rank Letter / NEET PG 2025 Score Sheet

Bond / Declaration (As per PG Prospectus 2025–26):

Candidates must execute two bonds of ₹200 each (on Kerala Stamp Paper – total value ₹200).

• Bond 1: Undertaking to serve the Government of Kerala for a minimum of 1 year after completion of the course.
• Bond 2: Agreement to pay liquidated damages (₹50,00,000) if the course is discontinued.

Bond Requirements:

• Sureties – 2 persons (parents of the candidate)
• Witness – 1 person (Gazetted Officer from Kerala State Service)
• All bonds must be submitted at the time of admission.

Soft Copy Submission

Students must submit soft copies of all original certificates and a passport-size photo along with the hard copies at the time of admission.
